APPOINTMENT INFORMATION
What happens during your first appointment
In your first 30 minute appointment Dr Pham will talk to you about your dental history and current concerns with the aim to:
- Listen to your dental worries and asses your dental needs
- Address any immediate pain or issues you may have
- Explain what will happen during your dental visit
- Inform you about the health of your teeth and mouth
- If necessary discuss your dental treatment plan options
- Discuss any fees related to your dental care to gain your approval before proceeding
- New patients will need to fill in a personal and medical history form on your first visit.

IF YOU REQUIRE IMMEDIATE DENTAL CARE:
If you have specific request for a service to relieve pain or discomfort, Dr Pham will assess your needs and where appropriate proceed with the service. We encourage you to have a comprehensive oral examination to ensure you know of the full extent of your dental condition and are educated about the options, services and costs associated with your dental care.
Your dental treatment plan
Dr Pham will develop a personalised dental treatment plan according to your needs and walk you through it to ensure you understand it and have the opportunity to ask any questions.
Only when you are happy with your dental treatment plan, approach and related costs will Dr Pham proceed with your dental treatment.
Dr Pham’s aim is to help you to:
- Control dental disease that destroys your teeth and gums
- Maintain functional mouth health for life long enjoyment and;
- Have great looking teeth so you are happy and confident to smile.
DR PHAM’S FIVE PROMISES – I will:
1. Provide you with a dental treatment plan (if needed).
2. Always give you an estimate of fees for your approval before I go ahead with any dental work.
3. Always look to provide you with affordable dental care – In special cases we will develop a payment plan for you if you are experiencing financial hardship.
4. Send you an appointment reminder via a text message or a phone call on the working day before your appointment – You will need to reply by text or phone to confirm your appointment.
5. Follow up your dental treatment (where necessary) to ensure you are happy with your progress and outcome.
Child Dental Benefits Scheme
If your children and eligible for the Child Dental Benefits Scheme please bring in the Medicare card with their details and fill in a Child Dental Benefits Schedule Bulk Billing Patient Consent form. You will need these details in order to access to the dental benefit scheme.

Veterans’ Affairs patients
We support our Veterans and welcome Veterans’ Affairs patients. Please ensure to bring Veterans’ Affairs membership card in order for us to make on-line claims for you.
Has your dentist retired recently
We would like to ensure we can provide you with the best dental care possible, so if your dentist has retired recently please ask for your dental records. This way Dr Pham can work hard to provide you with exceptional, continuing dental treatment.

Private health insurance
Claim with HICAPS
If you have private health insurance please bring your membership card with you. We have a HICAPS facility on-site that will allow immediate insurance payments.
We kindly ask that you pay any remaining fee at the time of your dental appointment.

You may also settle account with us and make your own claim with your private health insurance provider.
We are a Member First Provider of Bupa, HCF and Medibank Private and therefore members will receive their maximum benefit:
